COMPANY DESCRIPTION Nonprofit Healthcare Organization JOB DESCRIPTION The Interdisciplinary Team Manager (RN): Performs pre-authorization, discharge planning and review of inpatients with the IDT as stipulated by the guidelines and procedures outlined by the Center Operations Department Collects appropriate clinical information to make a determination of medical necessity for reviewed service based on Milliman Optimum Recovery Guideline and tracks variances Begins the discharge planning coordination process with the IDT upon admission or pre-certification Ensures that all assessments/reassessments are completed by the IDT members prior to care planning meetings Facilitates IDT meetings and care planning meeting to ensure IDT consensus in the development of member-centered care plan Communicates member's request or any clinical findings to the IDT for follow-up assessment/reassessment and care planning Contributes to department goals and objectives in maintaining a high quality medical delivery system Follows through with concerned IDT member(s) REQUIRED SKILLS Bachelor's degree in Nursing or related Health field 2+ years of direct patient care clinical experience 1+ year of Manager / Supervisor experience in an Inpatient, Home Care, or Managed Care Company Experience in a Certified Home Health Agency, Long-Term Home Health Care Program, or as a Care Manager CPT and ICD-9/10 coding knowledge Excellent communication skills Strong attention to detail DESIRED SKILLS Previous Care Management experience Certification in Case Management from an accreditation organization or a certification in a healthcare-related clinical fieldSalary Range: $90K - $110KMinimum QualificationLess than 5 years Associated topics: bsn, cardiothoracic, coronary, maternal, nurse clinical, nurse rn, recovery, registered nurse, surgery, unit
